SECTION 72

All declarations shall be established in four copies on forms furnished by the administrative P. 30 of 110 authority in charge of the Register of Cooperatives. The declarations shall bear the signature or fingerprints of the registrant or his authorized agent who shall show proof of his identity and, unless he is a lawyer or notary, the power of attorney signed by the registrant or on which is affixed his fingerprints. The first copy shall be kept by the administrative authority in charge of keeping the Register of Cooperatives. The second copy stating the date and the name of formality carried out shall be given to the registrant. The third and fourth copies shall be sent by the administrative authority in charge of the Register of Cooperatives to the National Registry for transmission of one of them to the Regional Registry.
