The subscription form shall state:

1) the name of the cooperative to be formed, followed, where necessary, by its acronym;
2) the amount of the subscribed capital, specifying the portion of cash contributions and that
of in-kind contributions;
3) the proposed address of the registered office;
4) the number of shares issued and their par value;
5) the name or business name and address of the subscriber as well as the number of shares
subscribed and payments made;
6) a statement of the depositary in charge of keeping the funds until the registration of the
cooperative at the Register of Cooperatives;
7) A statement of the delivery of a copy of the subscription form to the subscriber.
